Implement the new architecture documented at doc/developer/UserRolesArchitecture

This includes the former tickets:

  • #161 (Allowing the system to automatically re-try the request after the user has timed-out or hasn't logged-in yet.
  • #249 (Proper exception handler for all exceptions)
